PRIME-OncoTransition for Breast Cancer : A smooth hospital-community transition for people aged 65+ undergoing Post-tReatment preventIve MEdication. – PRIME-OncoTBC

Context: Women aged 65 and older undergoing long-term adjuvant endocrine therapy (AET) after breast cancer often experience persistent side effects, fragmented care, and insufficient support, leading to treatment discontinuation and reduced well-being. Despite partial solutions, no integrated, sustainable model has been widely adopted.
Objective: PRIME (Post-tReatment preventIve MEdication for the transition for Breast Cancer in Oncology) is an international research initiative designed to propose the most leading-edge, empowerment-based integrated survivorship care model for women aged 65 and older undergoing long-term AET. The objectives are to: 1) map and analyze existing integrated survivorship care models; (2) co-design a patient empowerment intervention; (3) pilot the intervention in 7 different environments; and (4) develop actionable recommendations for implementing and assessing an integrated survivorship model based on patient empowerment.
Methodology: The project’s methodology fits with the participatory intervention research and is based upon mixed methods (literature review, mapping, co-design, combination of qualitative and quantitative methods, consensus conference) in an interdisciplinary approach (medicine, public health, social sciences) and with a strong patient engagement design.
Conclusion: With its cross-country comparative approach and strong focus on co-construction and patient-engagement, PRIME aims to generate robust, transferable evidence on AET management for women aged 65 and older with breast cancer, and inform national and transnational policies in Europe, Switzerland, and Canada. By centering around the voices of women aged 65+, PRIME fosters equity, integration, and person-partnership in survivorship care, offering a solution to improve care and quality of life based on a novel empowerment integrated survivorship model.
