European Co-care Learning Ecosystems – CoCare

The CoCare project provides evidence-based pathways for a paradigm shift in primary and community care practices and policies, aiming to disrupt the traditional, fragmented, and provider-centric model by focusing on the upscaling of integrated, person-centred, and actor-empowering care ecosystems. In CoCare, existing care ecosystems encompassing diverse models of integrated care in five European countries are systematically identified and documented by utilising an ecosystem methodology.

These national cases provide illustrative examples how participatory approaches, person-centred innovation, and empowered individuals can contribute to a more integrated and responsive care system. Based on the micro-level evidence generated through the project's activities, a complexity-sensitive framework for European co-care learning ecosystems is developed to the scale-up integrated, person-centred care systems. The framework will encompass policy recommendations, best practices, and strategies for overcoming the challenges of larger scale implementation. The co-care learning ecosystem framework will also address the potential for cross-border collaboration and the development of common standards across European countries.

The CoCare project is expected to have a significant impact on the future of primary and community care in Europe. By promoting the adoption of integrated, person-centred co-care learning ecosystems, CoCare will contribute to improved health outcomes, efficiency of healthcare, and a more equitable and accessible healthcare system. The project will strengthen the role of community organizations and actors in providing essential support and services to individuals and families. The project will promote a more sustainable and resilient healthcare system by fostering collaboration among stakeholders, improving the efficiency of resource utilization, and addressing the challenges of an aging population with an increasing chronic disease burden.
